The Results of Minimum 5-Year Follow-up of CLS Stem in Total Hip Arthroplasty

Abstract
Purpose : To evaluate the clinical and radiological results of minimum five year follow-up of patients who underwent total hip arthroplasty (THA) with a cementless CLS stem (Cementless Spotorno, Protek, Berne, Switzerland).

Materials and Methods : Thirty-seven cases of THA with a cementless CLS stem were analyzed. The patients were followed up for a minimum of five years. Clinical analysis was done using the harris Hip Score, thigh pain. Radiological analyses were estimated by the radiolucency line, endosteal new bone formation, cortical hypertrophy and calcar atrophy, pedestal formation, stem subsidence, and osteolysis.

Results : The average Harris Hip Score was improved from 46.0 preoperatively to 93.1 at final follow-up. There was one case with mild thigh pain. In the radiological analyses, the rate of the radiolucency line was under 2.7% in regions 1, 2, 6 and 7, and under 20% in regions 3, 4, and 5. The rate of endosteal new bone formation was over 80% in regions 1, 2, 6 and 7, and 30~60% in regions 3, 4 and 5. The rate of cortical hypertrophy was 20~40% in regions 2, 3, 5 and 6. Calcar atrophy was observed in eight cases (21.6%) and pedestal formation was stable in four (10.8%). There was no evidence of stem subsidence of osteolysis.

Conclusion : The clinical and radiological results of using a CLS stem during THA were excellent after a minimum five years of follow-up. However, more long-term follow-up is necessary for complete evaluation.
